Mohawk was predeceased by his mother Donna Assiniboine and father Joey Chilly; kukums, Hortence Watcheston and Eileen Assiniboine; mushum Donald Assiniboine; and his brother Hughie Assiniboine.
He is survived by his children: Jordan, Derek, Raven Constant and Donovan Assiniboine; sisters, Donalda Assiniboine and Tanya Assiniboine, adopted sister Shellena Alexson and foster parents Winston and Bernadette Walking Bear.

A wake will be held on January 6, 2026, at 4:00 pm at the Chief Denton George Memorial Multiplex in Ochapowace First Nation. The funeral service will take place on January 7, 2026, at 2:00 pm, also at the Chief Denton George Memorial Multiplex. Interment will follow at 4:00 pm on January 7, 2026, at the Assiniboine Watchesten Burial Grounds on Ochapowace First Nation.
