He attended Granite Park middle School. He is survived by his mother Lisa Vadnais- Hernandez, his father, Guy William Hernandez (AZ), his brothers, Nick Black( UT), Cory James Hernandez(CJ), (UT), Toby Hernandez (AZ).
Jacob loved his Jordans and had acquired quite a collection which he was proud of! Jacob loved his family .He was very kind and generous and always had a smile which made anyone around him feel good and smile as well! He will be missed so much!!
