John was born October 10, 1931 in Wauwatosa, WI. He was a veteran of the US Army.
He was preceded in death by his mother, Phyllis Kubiak; and sister, Dorothy Jankowski.
He is survived by his son, John Kubiak, Jr. of Apex, NC; and cousin, Dolores Janus of Milwaukee, WI.
A memorial service will be held 3pm Sunday, June 7, 2015 at Apex Funeral Home Chapel, 550 W. Williams Street, Apex, NC 27502. The family will receive friends immediately following the service.
