Blake was born at Westlock, Alberta on May 11,1962 and is survived by his brother Casey of Calgary as well as several cousins, aunts and uncles throughout North America. Also leaving to mourn his loss are his best friends Melanie Roy and Greta Bylsma. Special thanks to Dr Kannen Osei-Tutu and nursing staff of unit 46 at the Special Services Building.
A Memorial Celebration of Blake’s life will be held on Wednesday October 26, 2011 at 1:30 p.m. at the Bow Park Court, (220 Scenic Bow Place NW) Recreation Room.
In Lieu of flowers, donations may be made to a society of your choice.
