Anissa Fields (nee Hernandez), 28 of Chicago was born in 1991 to the late Juan Carlos Hernandez and Lisa Hernandez and passed away on December 25, 2019. Anissa was the beloved wife of Michael Fields; loving mother of Mia Sofia and Odin Atlas Joaquin; dear sister of Anthony, Adam, and Alisia. A visitation will be held at Woodlawn Funeral Home 7750 W.
