She was born November 3, 1923, in Ambridge, PA. She was the daughter of the late Mildred Angel Reader and Harold Reader.
Hazel was preceded in death by her husband Edward Wozniak, Sr. and great grandson Charles Rowe.
She is survived by 2 sons and their wives. Edward Wozniak Jr. and his wife Donna of Center twp., and Richard Wozniak and his wife Tammy in Slippery Rock. Five grandchildren also survive Hazel. Natalie and Doug Rowe, Center twp.; Lauren and Ryan Sturgill, Virginia; Catherine and Edward Wozniak III, Baden; Richard and Jackie Wozniak, Virginia; and Sara Wozniak-Randall and Chris Randall, Washington. Six Great grandchildren survive Hazel as well; Maryn, Madelyn, Sam, John, Caleb, and Rebekah.
Hazel was a graduate of Allegheny School of Nursing. She was 2nd Lieutenant in the U.S. Army Nurse Corp. during WWII. Hazel was a member of Baden United Methodist Church. She retired from nursing in 1982.
There will be no visitation at the request of the Hazel.
