He is survived by his fiancée, Kim Brasher and her son Orion; parents, Ellis Slack and Marie Martinez and Revonda and Phil Ford; siblings, Natasha Slack, Jacob Slack, Rose and Elias Salazar, Robert Slack, Misty and Nate Wittke, Angela and Jeremy Hendrickson; grandfather, George Wolfe.
He was preceded in death by his grandparents, Carlyle and Marian Slack, and Grandmother Thelma Jean Wolfe and close cousin Valorie Watt.
A celebration of his life will be held on Sunday February 26, 2017 from 2:00-4:00 p.m. at Valley View Funeral Home, 4335 West 4100 South, West Valley City Utah.
