David W. Kotalik, 66, U.S.Marines Veteran, of Oak Park, passed away January 15, 2015, in the Seasons Hospice Inpatient Unit at Westlake Hospital. He was born in Chicago, IL to the late Joseph and Marie Kotalik. He is survived by his loving children Scott (Olga) Kotalik and Deborah (Hector Lopez) Kotalik, grandson Connor Lopez and siblings Joe (Louise) Kotalik and Barb (Robert) Massi.
