(Sandra, nee Post), Michael (Nancy, nee Hanson-Kolovitz), Terrence (Gail, nee Wallace), Brian, Kevin (Dee Dee, nee Balsamo), Trudi (Susan Kaszynski), Kathy Panek Goggin and the late Daniel; cherished grandfather of Kerry (Andrew Gallios), John (Megan, nee Ennessey), Sara (Brian Young), Michael (Kristen Eisenhart), Patrick (Bridget, nee Polacek), Maureen, Kelly (Jason Widuch), Brie Kolovitz, John McConachie, Terrence (Bridget, nee Leitz), Bridget (Joseph Bigane), Sheila, John (Katherine, nee Hill), Colleen (Thomas Krupa) Nicholas, Timothy, Matthew, Anthony; proud great-grandfather of 24. Jack was the longtime Associate Clerk of the Cook County Circuit Court, consultant to the Office of the Chief Judge of Cook County, retired member of Cook County Pension Fund Board of Directors, Retired Captain of the United States Army in China, Burma, and India Theatre, Past National Commander in Chief of the Military Order of the World Wars and Bailiff of The Military and Hospitaller Order of Saint Lazarus of Jerusalem. Visitation Tuesday 2 to 8 p.m. at Woodlawn Funeral Home, 7750 W Cermak Ave., Forest Park. Funeral Wednesday 9:15 a.m., from the funeral home, to St. Giles Church, Oak Park, Mass 10:30 a.m. Interment Mt. Carmel Cemetery.
