

Dr. Donn Slovachek, “Slov”, 63, of Newburgh, IN passed away Tuesday, December 8, 2015, at his home surrounded by his loved ones. He was born May 16, 1952 in Siren, WI to the late Jane C. (Olds) and Charles “Chuck” Slovachek.
Slov graduated from Menomonee Falls North High School in 1970. He attended the University of Wisconsin graduating Magna Cum Laude in 1975, University of Wisconsin Medical School in 1979, and joined the US Air Force. He served as Chief Resident during his tenure at Wright-Patterson Air Force Medical Center in Dayton, OH specializing in OBGYN in 1983.
His military career lasted eight years while stationed at Wright Patterson AFB and Mountain Home AFB. Dr. Slov went into private practice in Dickinson, ND, at the Dickinson Clinic and finished his ministry as an OBGYN at St. Mary’s Hospital in Evansville, IN. In January 2014, Dr. Slov left private practice to undergo an allogeneic stem cell transplant for treatment of high grade myelodysplasia (MDS).
He enjoyed many hobbies over the years including modified race car driving, hockey, hunting, fishing, riding his Harley, supporting the Packers, scuba diving as an Aquatech instructor and traveling with his wife.
Dr. Slovachek is survived by his wife of 30 years, Sharon (Lantern); his daughter, Susan Valentine; and his three step-daughters, Sonya (Ajay) Cropper, Stacie (Brick) Thompson, Sara (Heath) Hosick; many loving grandchildren, Kelsy, Bella, Tristyn, Ember, Mason, Kamdyn, Chloey, Gabriel, and Kinley; siblings, Dave (Patricia) Slovachek, Carol Slovachek, Laura (Mike) Gulatz, Ann (Dave) Mandick; several nieces and a nephew. He was preceded in death by his parents, Jane and Charles Slovachek.
The Slovachek Family would like to extend sincerest thanks to the following: Dr. Anthony Stephens, The Medical Staff at St. Mary’s, The IU Bone Marrow Transplant team, and Heritage Hospice for their exceptional care and services.
