Heather Horton Hernandez, of Machesney Park, Illinois, age 47, passed away on Monday, October 13, 2025 in Rockford, Illinois. She was born January 17, 1978 in Rockford, Illinois to Michael Horton and Renee Mancuso. She married Adam Hernandez on July 1, 2024.
Heather enjoyed being in nature. She liked swimming, fishing, and enjoyed going on adventures plus exploring the local areas. She loved to go antiquing with her family, even just window shopping, it was fun to see all the vintage things. She liked to watch movies with her daughter Angel. Heather loved to sit and talk with her loved ones. Her favorite animal were owls, she loved to try to listen for their hoots.
She is survived by her husband, Adam Hernandez; children, Paige (Karol Sawicki) Chabucos, Angel (Stormy Jose) Lundberg; and Matthew (Hanna) Lundberg; grandchildren, Savanna, and Wyatt Lundberg; mother, Renee (Chris) Mancuso; sisters, Julienne Huggins Rossman and Megan Vincent; along with two nephews.
She was preceded in death by her father, Michael Horton; grandmother, Cellus Grazioli; and brother, Michael Horton Jr.
In lieu of flowers, donations may be made to a local rehabilitation center in honor of Heather’s father.
A service for Heather will be held on Friday, October 24, 2025 at 2:00 pm at Advantage Funeral Home, 7000 W State St. Rockford, Illinois, 61102.
