Ted was preceded in death by his parents, Joseph Theodore Couillard II and Jean Elizabeth Maxwell and sibling, Belle Shukaliak.
He is survived by his son, Joseph Theodore Couillard IV and daughter-in-law Naheed Nizam of Seattle, WA, grand-children, Amir Couillard and Aliya Couillard, and siblings Paul Couillard and Alice Werenka.
