

She was born on August 28, 1940 in Oldham County. She was the second of four children to the late Claude Sr. and Agnes St. Claire Purvis.
In 1961, she married and raised three children.
Hazel worked at several hotels supervising housekeeping, then at Cedar Lake Lodge for several years. Later, she cleaned homes in the Louisville area.
Earlier, in the 1970's she won a state championship for skeet shooting at the Oldham County Sportsman Club.
She is preceded in death by her siblings, Claude Purvis Jr, Frances Powell, and James Purvis.
Left to cherish the memory of Hazel are her children, Bruce Cotton (Janet), Susan Hamilton, Leslie Cotton Gaddis; 5 grandchildren, J Todd Hamilton, Jessica N Hamilton, Jared N Cotton, Jeremy A Cotton, and Sheila Kay Gaddis; 4 great-grandchildren, Kaegan J Hamilton, DJ Terry Jr., Kyson M Cotton, and Laney Hamilton; a host of nieces, nephews, extended family, and friends.
A celebration of Hazel's life will be held at Heady-Radcliffe Funeral Home, 706 W Jefferson St, LaGrange, KY 40031 on Wednesday, August 27, 2025 at 3pm with burial to follow in Harrods Creek Cemetery in Brownsboro, KY. Visitation will be at the funeral home immediately before the service starting at Noon.
