Polanco, Jose Alfredo, of Westminster, died December 9, 2013 at St. Anthony Hospital. Services will be held from 1:00 to 3:00 p.m. on Friday, December 13, 2013 at the Carroll Lewellen Funeral Chapel, 503 Terry St. Longmont, CO. Services will conclude at the Chapel with cremation to follow.
