

Jake Michael Martiniak, age 30, of Hazel Crest, IL, passed away suddenly on Tuesday, November 4, 2025. Jake was the beloved father of his greatest joy, Layla Rose Martiniak. He was the loving son of Michele Martiniak (née Styler) and Michael (Maggie) Martiniak; cherished brother of Dick (Lisa) and Michael Jr. (Samantha); and a proud and playful “funcle” to Colton, Tyler, Levi, Cameron, Christian, Carter, and Cole. He was also a treasured grandson and a loyal friend to many.
Jake had a gentle heart and an unwavering kindness. He had a deep love for all animals and held a special compassion for those less fortunate. He never met a stranger—every person he encountered felt seen, welcomed, and valued. His laughter was infectious, his spirit warm, and his presence had a way of making life feel a little lighter.
A true Chicago sports fan, Jake could always be found cheering on the Bulls, Bears, and White Sox with passion and pride. But nothing in the world meant more to him than his daughter, Layla. She was the light of his life, and he dedicated himself to being the best father he could be. His love for her will forever be his legacy.
Jake will be deeply missed by all who knew and loved him. The world is a dimmer place without his bright smile and generous heart.
A memorial visitation will be held on Monday, November 10, 2025, from 4:00 p.m. to 8:00 p.m., with a memorial service at 6:30 p.m., at Blake-Lamb Funeral Home, 4727 W. 103rd St., Oak Lawn. Interment private.
