Jacob Richard Branch, 29, of Phoenix, Arizona, passed away on January 27, 2017. He was born in Stockton, California on June 4, 1987 to John and Carmen Branch. He is survived by his two children; his parents, John and Carmen Branch; his brothers; and numerous extended family and friends. A memorial service will be held at 1:00 pm on Saturday, February 4, 2017 at Green Acres Mortuary, 401 N. Hayden Rd., Scottsdale, AZ.
