Jerry died peacefully at home on August 10, 2014. Jerry was universally known for his kindness and compassion. He loved to cook for his family and his friends, whom he referred to as his family of choice. Jerry was devoted to Planned Parenthood of the Rocky Mountains, where he worked in the Human Resources and Accounting Departments for 23 years. He is survived by his mother, Connie Formhals, his brothers, Tom, Bob, Joe and Dave Formhals, his sisters-in-law, Helen, Maggie and Diane Formhals, and numerous and much loved nieces, nephews, grandnieces, grandnephews and cousins including his housemate Kathy Ebersberger. A service celebrating Jerry's life will be held on October 11, 2014, at 11 a.m. at Olinger, Crown Hill Mortuary, Pavilion of Reflection, in Wheat Ridge, Colorado.
